DENTAL INSURANCE PLAN Full and part-time employees are eligible to participate in the Dental Insurance Plan, which offers 2 levels of coverage and allows you to select the dentist of your choice. |

VISION INSURANCE PLAN Full and part-time employees are eligible to participate in the Vision Insurance Plan. The plan offers coverage for an exam and also provides coverage for hardware (frames, lenses, contacts, etc.). There is a large network of providers from which to choose. |

GROUP LIFE INSURANCE
Full-time employees are eligible to participate in a supplemental life insurance benefit. Employees can elect coverage up to 5x annual salary (with a maximum of $300,000), and coverage is available for a spouse and/or dependents as well. New hires enrolling within 30 days of hire are eligible for up to $100,000 of coverage ($20,000 for spouses) without needing to provide proof of good health. |

SHORT TERM DISABILITY PLAN Short Term Disability Plan Full-time employees are eligible for the company sponsored plan. Eligibility begins on the first of the month after 30 days of employment. The maximum benefit is 13 weeks of coverage. |
